Where do I find my state withholding and unemployment insurance account numbers?

To locate your withholding and unemployment insurance account information:

• Log in to the Agent dashboard at https://agent.middesk.com/.
• Click “Accounts.”
• Click on each state to review your related tax account details.

You will find your tax account numbers under each state agency’s title, e.g. Department of Revenue (DOR), Department of Labor (DOL), State Tax Commission, Department of Labor and Workforce Development, Employment Development Department (EDD), etc…
