Requirements vary state-by-state. In most cases, you will be asked to provide the following:

• Withholding Account Number
• Unemployment Tax Account Number

Depending on the state, you may also be asked to provide additional details, such as:

• Employee location information
• Federal Employee Identification Number (FEIN)
• Filing frequency
• Online portal credentials for each state agency you’ve registered with
• Unified Business Identifier (UBI)
